Oak Creek Ranch School, Sedona/Cottonwood, Arizona
Visiting Therapist
Dear Parents,
I was a visiting therapist at Oak Creek Ranch School and worked with students there for more than 18 years. I was on campus two days a week and families contract with me privately for services.
As I know you are aware, OCRS is NOT a therapeutic boarding school. However, many of our students have made good use of therapy at home in order to enhance their academics, social skills and overall wellbeing. Many students at OCRS never attended school away from home and sometimes had trouble adjusting to boarding school life.
I welcome your calls and am glad to discuss your child’s specific needs. I can most easily be reached by calling (626) 390-1458.
Respectfully,
Barbara Waldman, PhD
Office Hours at Oak Creek:
I am living full-time in Los Angeles and no longer work at Oak Creek. I stopped working there on my own accord and left on good terms.
Fee Schedule:
1. Individual Counseling for students is available on the school campus. All counseling is scheduled by appointment arranged during the academic week. This service is not a part of Oak Creek Ranch’s service. Payment is made directly to the therapist by credit card.
Therapy is billed at $160.00 per session unless other arrangements are made.
2. Written evaluations requested by the parents or the school. These may include mental health intervention summaries, intake reports and/or special evaluations pertaining to academic, social, intellectual, disability, or other need. $100.00 per hour for report preparation. Usual length of time involved in reports is a minimum of two hours.
3. Emergency Services. I am available to my clients and the school through my cell phone at all times.
4. There is a 24-hour cancellation policy. Non canceled sessions will be billed at the regular rate.
I do not accept insurance as payment for my services. Parents will be provided with a completed standard insurance form (HICVA) for services each month. It will be included in your monthly statement. It will be “insurance ready” for submission to most insurance companies.
Confidentiality:
The laws of confidentiality cover students and their families. Information will not be shared with Oak Creek Ranch School unless a signed consent is received from the parents if the student is under the age of eighteen. If the student is eighteen or older, the student may sign an authorization for release of confidential information. All authorizations for release of confidential information need to contain the following information: The name of the student, the names of the person requesting the release, clear and concise statement as to why the release is being requested, signature of the persons or persons authorized by law to initiate such a release, date of signing, witness to the signing, and a time frame for when the release is no longer considered valid. Confidentiality is waived if there is a threat of suicide with plan and intent, if there is the threat of homicide or harm to another with plan and intent, in all cases of child abuse which come to the attention of the therapist, if there is use of any substance which could permanently harm someone and by a court order signed by a Judge who is assuming responsibility for any aspect of the student’s or families legal situation.
Emergency Statement: The therapist is responsible as an outpatient therapist for counseling their clients on campus. The therapist assumes responsibility for intervening only with students they accept as clients. The therapist is not on call for Oak Creek School and is not hired or contracted with Oak Creek Ranch School. In case of an emergency, another therapist may be called and their intervention requested if available. Any other mental health professional in the community or through any mental health clinic or mental health hospital may also be called.
